Both editors and many of the contributors of this book hail from Johns Hopkins University and the University of Michigan. However, there are also quite a few authors from other nationally and internationally known programs, which lends this book a global appeal for those outside of the U.S. who want to learn how to practice state-of-the-art Pain Medicine. The explicitly stated audience for this book is non-pain specialist health care providers. As such, the editors recruited experts from different specialties to provide a multidisciplinary overview of how to treat various painful conditions. Given the high prevalence of chronic pain worldwide and the fact that chronic pain is the number one cause of disability, it is imperative to target those clinicians on the frontline who are most likely to see these patients first. This book provides a foundation on which the non-pain specialist can gain confidence in assessing and treating the patient with persistent pain. The book is divided into four major sections, namely Pharmacologic Pain Therapies, Nonpharmacologic Pain Therapies, Additional Considerations in the Pain Patient, and Case-Based Pain chapters. Chapters also are supplemented with rapid-access tables that highlight salient information on the selected topics. For example, the chapter “Opioids” lists common opioid side effects, propensity for developing tolerance, and how to treat the side effects. In the same chapter is a table listing equianalgesic doses, routes of administration, duration of action, and “pearls” associated with each opioid. The reader, however, should be cautioned on the use of opioid equianalgesic dosing tables in general. Because there is no one universally accepted equianalgesic conversion standard for all opioids, there is a substantial risk of inadvertently overdosing a patient by making a wholesale switch from one opioid to another all at once. Although the common practice of opioid rotation is undergoing serious reevaluation to improve outcomes and reduce inadvertent harms, for now it is best to use equianalgesic conversion tables only as a guide and gradually convert from one opioid to another over a few days to weeks to assess for efficacy and side effects. The reader will appreciate the case-based chapters, which provide clinical vignettes for exploring some of the more common diagnostic dilemmas commonly encountered in practice, such as low back, neck, and upper extremity pain; osteoarthritis; neuropathies; chronic postsurgical pain; postherpetic neuralgia; complex regional pain syndrome; primary headache; noncardiac chest pain; chronic functional abdominal pain; pelvic pain; central pain syndromes; and fibromyalgia. By using clinical vignettes, the reader can focus on reviewing the pertinent physical examination findings, diagnostic workup, differential diagnosis, and treatment for these common pain disorders. Although the book is lengthy, its size is intermediate between a pocket book and a traditional textbook. The wide-spaced lines and large text font make this book easy to read and access as an office practice reference. Therefore, target audience members can realistically expect that they can quickly implement the knowledge acquired from this book. The one minor deficit for this book, however, is the lack of an online edition. Overall, the editors and authors have done an excellent job in creating a book that has great appeal and value for non-pain specialist health care providers.
