- W2043498968 abstract "O87 Post transplant lymphoproliferative disorder (PTLD) that are considered malignant have traditionally been treated with chemotherapy. Little data exists regarding the efficacy of individual chemotherapy regimens for PTLD, and no individual drug regimen has been shown superior for the management of PTLD. Methods: All patients treated with chemotherapy for the diagnosis were examined for patient demographics, tumor characteristics recurrence and overall survival. Results: 193 patients with PTLD after solid organ transplantation managed with chemotherapy were identified. The majority of patients were male 142:51, Caucasian 148 (16 AA, 29 unspecified), B cell (81%), monoclonal (71). CD 20 was positive in 60% of patients tested. The distribution of organ recipients was: kidney 92 (K;48%), heart 54 (H;28%), liver 30 (L;16%), pancreas 8 (P;4%), and lung 9 (LU;5%). Mean age at diagnosis was 42.3 ± 18.8 yrs, with a median time to presentation of 24.5 mos (range 0.8 to 226.5 mos). Principal sites of disease were lymph node 92 (48%), GI tract 41 (21%), liver 39 (20%), allograft 35 (18%), lung 33 (17%), CNS 25 (13%), spleen 24 (12%), bone 20 (10%). Immunosuppression reduction was utilized in 40 (21%) and total discontinuation in 66 (34%) of patients. The multi-drug group consisted of four agent regimens dominated by a primary chemotherapy agent. The primary agents in pts treated with multi drug regimens were: (cytoxan; n=29, vincristine; n=12, methotrexate; n=11, adriamycin; n=8, and ARA-C; n=4).Figure*P<0.05 CHOP vs. Promace; ** P<0.05 CHOP vs. Single agent # P = 0.07 Conclusions: Single agent chemotherapy appears to be inferior to other chemotherapy regimens for PTLD in that its use is associated with a higher risk of death related to PTLD. The use of CHOP appears to demonstrate an increased interval to death, CHOP and Promace regimens demonstrate an improved survival compared to single agent regimens. The CHOP regimen appears to result in a high late recurrence rate." @default.
