- W2043606564 abstract "Increased intra-abdominal pressure (IAP) occurs in a variety of clinical situations such as accumulation of ascites, bowel distention from ileus or mechanical obstruction, and reduction into the peritoneal cavity of large, chronic hernia contents that have “lost their domain.” Intra-abdominal hypertension (IAH), or markedly increased IAP, is common after extensive abdominal trauma. The factors that contribute to IAH are accumulation of blood and clot, bowel edema or congestion from injury to mesenteric vessels or excessive crystalloid resuscitation, and perihepatic or retroperitoneal packing after damage control laparotomy for diffuse nonsurgical bleeding. Closure of the swollen and noncompliant abdominal wall under tension may further aggravate the IAH in these situations. IAH has profound effects, both abdominal and systemic. These physiologic aberrations, in concert, may result in organ dysfunction and failure. It is therefore important to anticipate IAH, attempt prevention, recognize its presence, and establish treatment. Sometimes IAH is referred to as the abdominal compartment syndrome (ACS). This syndrome is characterized by a tensely distended abdomen, elevated intra-abdominal and peak airway pressures, inadequate ventilation with hypoxia and hypercarbia, disturbed renal function, and an improvement of these features after abdominal decompression. This article discusses the physiologic consequences of IAH and focuses on its prevention and treatment. We also emphasize the concept that IAH and ACS are not synonymous and that ACS is a late manifestation of uncontrolled IAH." @default.
