- W2045805000 abstract "Background: Gender differences exist in the treatment of a number of cardiovascular diseases, and studies suggest standard medical therapy and cardiac procedures may be underutilized in women. There are very few studies examining gender differences in the care of patients with CHF. The purpose of this study was to determine if a gender gap exists in therapy and procedure use in patients with CHF in Quebec, Canada. Methods: The Quebec hospital discharge summary database (Med-Echo) was linked with the provincial physician and drug claims database (Regie de l'Assurance Maladie du Quebec [RAMQ]). Patients were included in the current study if they were admitted to hospital with a diagnosis of CHF (International Classification of Disease 9 [ICD9] code 428) between January 1, 1998 and December 31, 2002 and had no previous admission for CHF in the 3 years prior to hospitalization. Results: There were 16017 men and 16622 women in the cohort. Women with CHF, when compared to men, were older (78±11 vs. 73±11 years, p<0.001), had more hypertension (41% vs. 28%, p<0.001) and hyperlipidemia (18% vs. 14%, p<0.001). Women were less likely to see a cardiologist (30% vs. 34%, p<0.001), and required a longer hospital stay (12.0±14.8 vs. 10.6 ±13.3 days, p<0.001). During the first year after hospitalization, women were less likely to have an assessment of left ventricular function (61% vs. 65%, p<0.001), diagnostic cardiac catheterization (11% vs. 15%, p<0.001), and revascularization procedure (4% vs. 6%, p<0.001). Women were less likely to be taking ACE inhibitors after CHF hospitalization (60% vs. 66%, p<0.001), and more likely to be receiving beta-blockers (38% vs. 34%, p<0.001). Men and women had similar yearly rehospitalization rates (1.5±1.0 vs. 1.4±1.0, p = 0.7). Cumulative 1 and 3 year survival rates were similar for men and women (69%/53% vs. 68%/52%, p = NS). Gender did not impact the prognostic value of comorbidities known to be associated with an increased risk of death in CHF patients, including diabetes, renal failure, and myocardial infarction. Conclusions: Elderly women represent a significant proportion of patients admitted with CHF and may represent population of patients that is treated suboptimally. Elderly women with CHF are less likely to see a cardiologist, less likely to have related cardiac procedures and less likely to receive standard medical therapy for CHF. Despite these findings, clinical outcomes are similar. This discrepancy requires further investigation, possibly with more detailed analysis of CHF etiologies." @default.
