- W2045831778 abstract "BackgroundThe CYP2D6 gene duplication leading to ultrafast metabolism might be one reason for treatment-resistance to antidepressant drugs being substrates. We tested the influence of the CYP2D6 gene duplication on pharmacokinetics and -dynamics of the tricyclic antidepressant doxepin in 11 healthy ultrafast metabolizers compared to extensive and poor metabolizers.MethodsE- and Z-Doxepin and E- and Z-N-desmethyldoxepin plasma concentrations were measured 0–72h after intake of a single dose of 75 mg doxepin. 5-HT-uptake and concentration in platelets were measured during the first 4 hours as surrogate parameters for antidepressant activity.ResultsCYP2D6 activity had a significant influence on both, pharmacokinetics of doxepin as well as on N-desmethyldoxepin. A linear gene-dose effect of the number of active CYP2D6 genes was detected for E-doxepin with AUCs of 235, 424, 1058, and 2730 nmol · h· L-1 in ultrafast, extensive, intermediate, and poor metabolizers. 5-HT concentrations in platelets showed a significant trend towards higher concentrations in carriers of genotypes with higher CYP2D6 activity whereas no effects of doxepin on 5-HT-uptake were detected.ConclusionThe CYP2D6 gene duplication lead to about 50% reduced mean AUCs of the active E-doxepin.
