- W2046376039 abstract "Several studies have demonstrated that an aerobic endurance training program in cardiac patients results in an improvement in maximal physical working capacity. It is unclear, however, whether these improvements are really based on an increased aerobic capacity or other factors like a higher motivation or increased self-confidence. Therefore, we investigated 301 men and 43 women with coronary artery disease (CAD) (age: 55.5 ± 10.0 yrs) before and after a total of 18.4 ± 1.9 exercise units on a bicycle ergometer which they performed during a 4-week ACR. Absolute performance and work loads at 4 predefined lactate levels were assessed during incremental bicycle ergometry. Mean maximal performance increased significantly (103.3 ± 34.0 vs. 118.5 ± 39.9 watt, p < 0.0001). Work load at the 1.5 mmol lactate level improved from 56.3 ± 20.3 to 68.7 ± 26.7 watt, at the 2.0 mmol/l level from 70.3 ± 23.2 to 83.7 ± 25.6 watt, at the 2.5 mmol/l level from 82.7 ± 22.8 to 96.0 ± 26.8 watt, and at the 3.0 mmol/l level from 92.1 ± 23.5 to 105.3 ± 25.9 watt, p < 0.0001, respectively. Heart rate remained unaltered at the 1.5. 2.0. 2.5 and 3.0 mmol/l lactate level. These data clearly demonstrate that the improvements in maximal exercise capacity are really based on better aerobic endurance capacities, and are not only based on increased motivation or self-confidence. This is supported by the observation that despite improvements in performance at all predefined lactate levels the heart-rate remained unaltered. These findings indicate that the regular aerobic training during the cardiac rehabilitation program economises the cardiovascular system resulting in a decreased heart-rate on defined workloads. The effects were independent of pharmacological influences (e.g. ß-receptor antagonists). These findings are of high clinical importance with respect to the intended reduction of myocardial oxygen consumption in patients with CAD after a rehabilitation program." @default.
