- W2046529471 abstract "Patients with cirrhosis often show an inability to maintain a normal size of the extracellular fluid volume, which results in the accumulation of large amounts of fluid in the peritoneal or pleural cavities and interstitial tissue.5, 41 This abnormal extracellular fluid volume regulation is associated with alterations in renal function. In most instances, the predominant abnormality of renal function is sodium retention, and its main clinical consequence consists of recurrent accumulation of extracellular fluid as ascites, pleural effusion, or edema. In other instances, however, the derangement of kidney function is more severe, and results in an impairment of water handling or renal vasoconstriction. The main clinical consequences of these abnormalities are dilutional hyponatremia and hepatorenal syndrome, respectively. Finally, the existence of ascites may lead to another common and severe complication of cirrhosis: spontaneous bacterial peritonitis. In this article, the pathophysiology and treatment of ascites, dilutional hyponatremia, hepatorenal syndrome, and spontaneous bacterial peritonitis are reviewed." @default.
