- W2046751388 abstract "Aim of this study was to asses the effect of aliskiren additino to amlodipine on ankle foot voume (ADV) and pretibial subcutaneous tissue pressure (PSTP), two objective measures of ankle oedema. After a 4-week placebo period, 50 mild to moderate hypertensive patient (DBP>90 mmHg and < 110 SBP> 140 mmHg), were randomized to amlodipine 10 mg or aliskiren 300 mg or amlodipine 10 mg plus aliskiren 300 mg for 8 weeks according to a cross-over design. At the end of the placebo period and of each treatment period, blood pressure, AVF and PSTP were evaluated. AVF was measured using the principle of water displacement. PSTP was assessed connecting the subcutaneous pretibial interstitial environment with a water manometer. Both amlodipine and aliskiren monotherapy significantly reduced SBP (- 15,8 and – 14,9 mmHg, respectively, p <0.001 vs baseline) and DBP (-12.7 and 11.8 mmHg, respectively, p< 0.001 vs baseline) but the reduction was greater with tbe combination (- 23.3 mmHg for SBP, p< 0.001 vs monotherapy; -15.9 mmHg for DBP, p< 0.01 vs monotherapy). Amlodipine monotherapy significantly increased both AFV (+ 22%, p< 0.001 vs baseline) and PSTP (+ 66.3%, p< 0.001 vs baseline) while aliskiren monotherapy did not influence them. As compared to amlodipine alone, the combination produced a less marked increase in AFV (+ 6.7%, p < 0.01 vs amlodipine) and PSTP (+ 25.2%, p < 0.01 vs amlodipine). Ankle oedema was clinically evident in 16 patients with amlodipine and in 4 patients with the combination. These results suggest that direct rennin inhibition partially counteract the microcirculatory changes responsible for calcium channel blockers induced oedema formation." @default.
