- W2046808101 abstract "Taurine conjugated bile acid is a putative bile acid mediating intrahepatic cholestasis of pregnancy (ICP). The etiology of adverse fetal outcomes related to ICP is poorly understood; however, based on an animal model, it is hypothesized to be associated with impairment of cardiomyocyte function secondary to derangement of calcium dynamics. It is unknown if any of these effects manifest in the fetal vasculature. The objective of this study was to determine if continuous infusion of taurocholic acid into the fetoplacental and intervillous circulation of a placental cotyledon affects the fetal arterial pressure response after injection of a thromboxane mimetic U46619. We selected five placentas from normal, unlabored, term patients. Two cotyledons from each placenta were isolated and dually perfused. Continuous infusion of taurocholic acid at concentrations found in patients with ICP was performed into the fetoplacental and intervillous circulation. After 30 minutes, a bolus dose of U44619 was injected into both the test and control circuits. Pressure excursions were measured and compared to baseline pressures using a paired student's t test. There was significant attenuation of the pressure excursion in the cotyledons perfused with taurocholic acid compared to control after injection of U46619. The difference from baseline in the taurocholic cotyledon was 44.2 mm Hg vs 71.8 mm Hg in the control (p=.009). The perfusion of taurocholic acid attenuated the pressure response to thromboxane mimetic U44619 in the fetoplacental arterial circulation of a placental cotyledon compared to control. We hypothesize that this attenuation may be due to similar impairment found in cardiomyocytes. This finding in our ex-vivo model may represent the changes that occur in the placental vasculature with intrahepatic cholestasis of pregnancy. These placentas may have dysregulated vascular tone, leading to an impaired ability to vasoconstrict during times of fetal stress." @default.
