- W2047546553 abstract "Purpose of review In addition to psychosocial disability, severe mental illness is associated with alarmingly high medical comorbidity and mortality rates. This review provides a 5-year update (1999-2004) on the medical needs of patients with severe mental illness, barriers to medical and preventive care and interventions aimed at improving medical outcomes. Recent findings Physical illness is responsible for most excess deaths in patients with schizophrenia and bipolar disorder; circulatory disease is the principal cause. Diabetes and obesity are more prevalent among schizophrenia patients, partly resulting from antipsychotic use. High rates of smoking, unhealthy diets and lack of exercise are more common than in the general population. In part, increased mortality among patients with severe mental illness has been attributed to substandard medical care that they receive. Barriers to treatment may include separation of medical and psychiatric care, provider bias, cognitive impairment and non-adherence. Different approaches to integrating primary care into the mental health system have been suggested: provision of medical and preventive care by psychiatrists and collaboration with on-site medical practitioners. Summary Primary care and prevention must be emphasized in comprehensive treatment of patients with severe mental illness. There is an urgent need for more research into integration of medical and psychiatric care and new approaches to improving health outcomes in people with chronic mental illness.
