- W2047649969 abstract "The operation is safe and bone preserving for selected young patients with ACL deficient medial compartment osteoarthritis. The sequelae of the medial menisectomized, ACL deficient knee often leads to symptomatic medial osteoarthritis and ACL deficiency in many active patients. Current options include valgus tibial osteotomy with or without ACL reconstruction, and total knee arthroplasty. Unicompartmental replacement is generally contraindicated in this scenario. We present early results in a series of 9 such patients who underwent concomitant ACL reconstruction and unicompartmental Oxford knee replacement. Eight females and 7 females with median age 53.1 yrs (range 44 - 71) were included in this consecutive case series review. Patients were included if they had a KT side to side difference of > 5 mm and a positive Lachmann in addition to symptomatic medial compartment arthritis. The first patient had staged procedure, with ACL preceding the Oxford by 4 months. All 14 others had concomitant ACL reconstruction and Oxford hemiarthroplasty. Surgery proceeded as follows 1.Arthroscopic evaluation, autogenous hamstring harvest with graft preparation, 2. ACL tunnel creation, (anteromedial portal for femoral tunnel), graft passage and femoral fixation with Endobutton 3. Standard Oxford bone cut preparation, and cementation with trial spacer. 4. Tibial fixation of ACL graft 5. Insertion of definitive spacer. Patients were assessed at a mean of 31 months (range 12 - 60 months). WOMAC scores have all shown significant improvement from baseline and 7 of 9 KT measures were less than 5 mm side to side. There were no infections or bearing dislocations. There was one revision to total knee arthroplasty because of femoral loosening. Most patients were particularly satisfied having avoided total knee arthroplasty. Combined ACL reconstruction and Oxford hemi arthroplasty of the knee can be performed safely with generally good satisfraction. Nevertheless, this procedure is best considered experimental until medium term data support its use and patients should be advised of such." @default.
