- W2049372969 abstract "Gonorrhea has been among the most widely studied diseases in the past 3 decades in its clinical epidemiological diagnostic therapeutic and host-parasite aspects. The bacterium Neisseria gonorrhoeae is an exclusively human parasite. Gonorrhea infects the mucosa of the genital tract rectum and pharynx depending on sexual practices. It produces uncomplicated infections or acute suppuration that may culminate in tissue invasion with chronic inflammation and fibrosis. It may produce infertility in both men and women. An estimated 20% of women with gonococcal salpingitis become sterile. N. gonorrhoeae as a genitourinary pathogen is capable of colonizing the surface mucosa of the genital tract living in conditions of limited availability of iron and evading the hosts immune response. Bacteriological diagnosis requires an adequate sample determined by the patients sexual practices or cutaneous biopsies when dermatitis is present. Ophthalmic samples are generally obtained from newborns. Diagnosis may often be made from direct observation of male urethral specimens. Serological tests have been developed and two tests based on techniques of molecular biology are available. It has been estimated that maintenance of endemic gonorrhea in the population requires an average of 4 sexual partners per year per infected person. Transmission by asymptomatic persons is significant. Risk of infection has been estimated at 20% for each exposure of infected women to the male urethra and at 50% for each male-to-female contact. Treatment for nonresistant strains of N. gonorrhoeae continues to be based on penicillin. Prevention requires sex education to discourage sex with strangers and with numerous partners. Latex condoms and chemical microbicides during coitus are advised." @default.
