- W2049629053 abstract "Introduction In general, skin metastasis from internal tumors are uncommon and among them, cutaneous metastases from esophageal adenocarcinoma (EADC) are exceedingly rare. It is even more uncommon and unusual to have a patient with esophageal adenocarcinoma whose initial presentation was a facial skin nodule. Case presentation Our patient is a 76- year-old Caucasian male who presented a painless, indurated, non-mobile, 1.5 cm skin nodule above the superior lip region. According to the patient, he had noticed the lesion two months previously. However, in the past four weeks he complained of a mild cough and dysphagia. A skin biopsy was performed, which disclosed metastatic adenocarcinoma cells in the dermis with characteristic signet-ring cells. Subsequent investigations with computer tomography detected the primary tumor in the lower third of the esophagus and metastatic disease in the paratracheal lymph nodes. Biopsies of the esophageal mass after esophagoscopy confirmed the presence of infiltrated adenocarcinoma-poorly differentiated with characteristic signet-ring cells. Morphologically and according to an immunohistochemical comparison, the skin lesion and the primary tumor were identical. Discussion Skin metastasis from internal malignancies are estimated at 0.7–9%. The skin is rarely involved by metastatic tumors. Cancer types most commonly associated with cutaneous metastasis are breast, lung and melanoma. The most common sites of skin metastases are on the chest and abdomen. Cancer of the esophagus constitutes 7% of all gastrointestinal cancers. It is the ninth most common malignancy and the sixth most frequent cause of cancer death in the world. When skin metastases occurs in esophageal carcinoma, they usually involve the esophageal squamous cell carcinoma. Conclusion We reported a patient who was diagnosed with EADC, after presenting with an isolated metastatic facial nodule. Both, the presentation, as well as the metastatic pattern of this case, are unusual. Early biopsy of suspicious skin lesions is important and can reveal the existence of distant malignant disease, leading to diagnosis and appropriate treatment." @default.
