• W20509600 abstract "In 11 handelsueblichen Mohnmischungen wurden Opiatalkaloide nachgewiesen: Morphin bis zu 87,5 myg/g Codein bis zu 16,0 myg/g, Noscapin bis zu 15,7 myg/g und Papaverin bis zu 0,057 myg/g. Nach Verzehr von 1/2 - 3 Stuecken eines daraus hergestellten Mohnkuchens (10 - 60 g Mohn) waren in Urinproben, die bis zu 48 h nach der Mohnaufnahme abgenommen wurden, positive immunchemische Opiatbefunde zu verzeichnen. Mittels HPLC-MS/MS wurde Morphin-3-Glucuronid neben Morphin-6-Glucuronid als Hauptmetabolit detektiert, mit einer Maximalkonzentration nach 8 h von 20,3 myg/g. Freies Morphin war nach 4 h mit maximal 0,77 myg/g aufzufinden. Die Gesamtmorphin-Konzentrationen nach 4 - 6 h lagen bei maximal 17 myg/g und 5,7 myg/g an Gesamtcodein. In Blutproben, die bis zu 6 h nach der Ingestion abgenommen wurden, wurden immunchemisch Befunde im Bereich des cut-off-Wertes erhalten, wohingegen in allen Proben mittels HPLC-MS/MS beide Morphin-Glucuronide detektiert wurden. Freies Morphin wurde in Konzentrationen zwischen 1 und 3 ng/ml aufgefunden. Von den 6,5 Stunden nach der Aufnahme durchgefuehrten 6 Opiat-Schnelltesten auf Schweissbasis ergaben 3 Teste einen positiven Befund. Weder im Blut noch im Urin wurden Noscapin oder Papaverin detektiert, die als Marker fuer einen Konsum von Strassenheroin angesehen werden. Auch wenn nach den hier erhaltenen Befunden diese Begleitalkaloide nicht nach dem Verzehr von Mohnprodukten aufgefunden wurden, so wurden sie dennoch in Mohnprodukten detektiert. Aus diesem Grund sollte man auch bei unauffaelligen Urinbefunden zurueckhaltend in der Beurteilung sein. Neben 6-Monoacetylmorphin als allgemeinem Heroinmarker ist nur Acetylcodein als letztendlich definitiver Marker eines vorausgegangenen Konsums von Strassenheroin anzusehen. (A) ABSTRACT IN ENGLISH: Opiates were detected in eleven commercially available poppy seed products: Morphine concentration went up to 87.45 myg/g, codeine up to 16.0 myg/g, noscapine up to 1.57 myg/g, papaverine up to 0.057 myg/g. After consumption of 1/2 - 3 pieces of home made poppy seed cake (10 - 60 g of poppy seeds) positive immunoassay analyses were obtained up to 48 hours later. Morphine-3-glucuronide and morphine-6-glucuronide were detected by HPLC-MS/MS analysis, reaching concentrations of up to 20.3 pg/ml urine. Maximum concentration of free morphine was 0.77 pg/ml, with total morphine concentration reaching 17.0 pg/ml, and total codeine concentration of up to 5.7 pg/ml was detected 4 - 6 hours after consumption. Blood samples were taken for 6 hours following ingestion, immunoassay analysis showed positive results in the range of the cut-off levels, whereas both morphine glucuronides were found in HPLC-MS/MS analysis, free morphine was also found in concentrations ranging from 1 to 3 pg/ml. Six sweat tests for opiates were used 6.5 hours after intake, 3 tests showed a positive result. Neither noscapine nor papaverine, two markers for the consumption of illicit heroin, could be detected in blood. Although these markers were not found in body fluids they were found in the different poppy seed products. Therefore, negative diagnostic findings should be assessed carefully. Beside 6-acetylmorphine as general marker for heroin use, 6-acetylcodeine should be regarded as the definitive marker for consumption of street heroin. (A) Auszugsweise vorgetragen auf der 83.
