- W2051210668 abstract "The data are clear: meningococcal disease is rare in travelers, but it is a devastating disease when it does occur. 1 The course of the disease is often fulminant, with a very narrow time window between diagnosis and treatment. This makes the prognosis worse in travelers to remote areas with limited or delayed access to highquality medical care. Even with timely and appropriate treatment, case-fatality rates are high (10%–14%) and up to 20% of survivors suffer serious permanent sequelae. The estimated incidence in travelers varies widely, between 0.04 and 640 per 100,000 depending on destination. 2,3 Compared with yellow fever, with a reported incidence between 0.05 and 50 per 100,000 travelers, meningococcal disease occurs more frequently. 1 Yet, travel medicine providers and travelers alike have a much higher awareness about the need for vaccination for yellow fever than for meningococcal disease. Meningococcal disease also differs from yellow fever in another aspect. Although yellow fever is a disease only at destination countries, limited to Africa and the Americas, meningococcal disease is a worldwide problem. Immunization against meningococcal disease will not only protect during travel, but also protect individuals in their home countries. For example, the overall annual incidence rate in the United States and in countries in the European Union is currently 0.3 to 8.9 per 100,000 population. 4,5 Indeed, routine immunization against meningococcal disease is now a standard recommendation in the United States, most European countries, Australia, and New Zealand. The Advisory Committee on Immunization Practices (ACIP) in the United States recommends quadrivalent meningococcal conjugate vaccine for all persons aged 11 to 18 years regardless of travel destination.
