Their benefits must be weighed against well known deleterious side effects, which include Cushingoid appearance, osteoporosis, cataract formation, myopathy, hypertension, and, in children, retardation of growth and sexual maturation. Endogenous hypercortisolism has detrimental psychologic consequences that were first noted in the original observations of Harvey Cushing. Depression and paranoid ideation, the emotional disturbances most commonly associated with Cushing's disease (1, 2), cease when the underlying metabolic disorder is corrected (3). Exogenously administered corticosteroids are known to cause iatrogenic psychologic side effects in some patients, but few reports of such cases have appeared in the general medical literature.Psychiatric morbidity caused by adverse drug reactions is not a problem commonly addressed by clinicians. Yet, a survey of drug side effects in general practice revealed that neuropsychiatric reactions constituted 30% of all cases (4), and the Boston Collaborative Drug Surveillance Program (BCDSP) reported that 2.7% of hospitalized patients receiving nonpsychiatric drugs experienced adverse psychiatric reactions (5). Establishing the cause of a drug reaction is always a difficult task, one that is compounded in the case of neuropsychiatric reactions that may be delayed in onset and may persist beyond the discontinuation of the inciting medication. Further confounding factors include placebo effects, intercurrent illness, and potential psychiatric consequences of the disease under treatment. Because these variables cannot be controlled, most studies rely on establishing a close association between the abnormal findings and the time of drug administration and/or its concentration in the plasma.
