- W2051236972 abstract "There is clear evidence from two systematic reviews that radiotherapy reduces the risk of local recurrence in patients with rectal cancer, but the data on survival are still equivocal. To assess the effects of chemotherapy combined concomitantly with radiotherapy in the reduction of all-cause mortality and in the increase of disease-free survival, local, and distant control. Computerized bibliographic searches of MEDLINE and CANCERLIT (1970-2007) were supplemented with hand searches of reference lists. Studies were included if they were randomized controlled trials (RCTs) comparing mortality of chemotherapy combined concomitantly with preoperative or postoperative radiotherapy to preoperative or postoperative radiotherapy alone, and if they included patients with histologically proven rectal adenocarcinoma without metastases. Twelve eligible RCTs, 6 of preoperative chemoradiotherapy versus preoperative radiotherapy (2,590 patients), 4 of postoperative chemoradiotherapy versus postoperative radiotherapy (697 patients) and 2 of postoperative chemoradiotherapy versus preoperative radiotherapy (257 patients), were identified and included in the meta-analysis. Data on study populations, interventions, and outcomes were extracted from each RCT according to the intention to treat method by three independent observers and combined using the DerSimonian and Laird method. Chemotherapy concomitantly delivered with preoperative or postoperative radiotherapy compared to preoperative or postoperative radiotherapy alone significantly reduced the 5-year (OR 0.69; 95% CI, 0.50-0.93; p = 0.02) local recurrence rate. No reduction was observed in the reduction of 5-year mortality rate (OR 0.96; 95% CI, 0.81-1.14: p = 0.63), in the reduction of 5-year cancer-related mortality rate (OR 0.85; 95% CI, 0.64-1.14: p = 0.29) and in the occurrence of distant metastases (OR 0.99; 95% CI, 0.85-1.16; p = 0.91). Chemotherapy concomitant to radiotherapy significantly increase the local control of disease in patients with resectable rectal cancer, which may give a better quality of life to patients." @default.
