- W2051430210 abstract "The biologically active vitamin-D metabolite 25-hydroxycholecalciferol (25-H.C.C.) was given to two children with hypo-phosphatæmic (vitamin-D-resistant) rickets (H.R.) and to two normal subjects, and its actions on urinary electrolyte excretion, serum calcium and phosphorus concentrations, and circulating levels of parathyroid hormone (P.T.H.) were observed. In both children with H.R. the daily oral administration of 2500-5000 units of 25-H.C.C. resulted in an immediate and sustained fall in urinary sodium and potassium excretion of more than 40%, which was not produced by variations in the filtered load of these ions or by alterations in serum-calcium or immunoassayable P.T.H. levels. Urinary phosphate excretion remained unchanged, but urinary calcium declined significantly in one of the two patients. 25-H.C.C. did not alter urinary or serum electrolyte levels in the two normal subjects. This is the first demonstration of a sustained direct effect of vitamin D on renal tubular electrolyte transport in the human kidney. The results suggest that in patients with H.R., either the (proximal) phosphate transport pathway is absent or defective, or some other tissue-active vitamin-D derivative is not produced in sufficient quantity to effect renal tubular reabsorption of phosphate." @default.
