- W2051571519 abstract "With a touch of sadness, but a feeling of satisfaction, I write my last “State of the Journal” column for HeartRhythm. Winds of change, not devastating like Katrina and Sandy, but a refreshing Caribbean breeze, create and renew. We began HR in May 2004 with 700 submissions and an acceptance rate of 25–30% the first full year; we will finish in December 2013 with over 1500 submissions and an acceptance rate of 15–17%. We published about 1400 manuscript pages and 180 original articles initially, increasing to about 1800 pages and 250 original articles last year. Throughout this growth, however, reviewers' turnaround times remained 7–9 days and≤10 days to the first editorial decision. We were among the first of Elsevier's 1500 journals to institute electronic publication 3–5 days after article acceptance, now widely adopted by many journals. HeartRhythm rapidly became a leader among the EP journals, respected by electrophysiologists around the world. HeartRhythm has benefited, some might say suffered, from very tight editorial control, so new leadership will be invigorating. Surely things will change but I am certain the new editor will continue to embrace both clinical and basic science and will have lots of innovative ideas. Associate editors will likely play increasing roles to handle the manuscript load. The transition will occur gradually during the last half of 2013, with the full shift January 1, 2014. As many of you can attest from personal experience, the acceptance of case reports in HeartRhythm has been very difficult, with an overall acceptance rate of only 4%, and about half the articles rejected without outside review. One reason is that case reports adversely affect the impact factor. Nevertheless, many case reports are interesting, fun to read, and have some educational value. Recognizing this, the Heart Rhythm Society has decided to launch a journal devoted solely to case reports, hopefully some time this year. As of this writing, details are still being worked out, but the new journal will be online only, and will interact with the “mother” journal in some way yet to be established. We are excited about this new initiative and applaud the decision. As I have published before, the HEART (Heart Editors Action Round Table) Group Editors is made up of editors from almost 100 cardiovascular journals worldwide and meets three times a year to discuss issues of mutual interest, such as ethical publishing, problems of plagiarism, and so forth. Most recently, led by Chris Cannon, the HEART Group Editors has considered a standardized approach for matching language to the level of evidence when describing outcomes data from observational versus randomized controlled trials (RCT). We agreed that wording should convey to the reader how certain the conclusions are and should be different for the different kinds of trials. So, for example, results from an observational study should have descriptive terminology such as “the therapy was associated with a reduced mortality,” while results from a RCT could state more definitively, “the therapy reduced mortality.” The document from that deliberation is published as supplementary material on the HR website and serves as a template for authors of future articles. This model is being adopted by virtually all member journals and I strongly suggest that wording from that article be used in the future. Joan and I have had the very good fortune of working with outstanding people most of our professional lives, and the last 10 years (25 if I go back to starting JCE in 1989) in our roles as editor and executive editor have been no different. We owe so much of HR's success to the staff and leadership of HRS, to our publishers at Elsevier, but most of all to our readers, reviewers, and authors. It has been a great ride, and we thank everyone for making it so enjoyable. We mourn the loss of several colleagues over the past year, and welcome the new members of the editorial board; and so the never-ending cycles continue.
