• W2051771838 abstract "아편계 약물들은 다른 부수적인 효과와 함께 강력한 진 통 작용을 갖고 있다. 이런 아편계 약물의 반복적인 사용은 진통 효과에 대한 내성(tolerance)을 보이며, 약물에 대한 육 체적 의존성(physical dependence)을 나타나게 한다. 수 십 년 동안 아편계 약물 분야의 연구에 다양한 발전 이 있었다. 그 동안의 연구 결과, 내인성 아편계 펩타이드, 다양한 아편계 약물 수용체 및 아편계 약물 유사 수용체 등이 밝혀졌으며, 아편계 약물의 작용 및 내성에 대한 분자 생물학적 기전의 연구에도 상당한 진전이 있었다. 아편계 약물은 G 단백 수용체(G protein-coupled receptor, GPCR)의 일종인, 고유의 수용체(μ, δ, κ)를 통한 신호 전 달 기전을 통해 그 작용을 나타낸다. 신경 세포에서 수용체 가 자극되면, G 단백 결합체가 활성화되어 Gα-GTP와 Gβγ 결합체로 나누어진다. Gα-GTP는 adenylyl cyclase의 작용을 억제하여 cyclic adenosine monophosphate (cAMP) 형성을 저하 시킨다. Gβγ 결합체는 G protein-activated inwardly rectifying potassium channel (GIRK)을 활성화시키고, voltage-dependent calcium channel (VDCC)을 억제시켜, 접합전 과분극(presynaptic hyperpolarization)을 초래하며, 흥분성 통증 전달 물질 유리를 억제하여 결과적으로 통증 억제 작용을 나타낸다(Fig. 1). 아편 내성의 기전은 더욱 복잡한데, 정리하면 다음과 같 다. 첫째, 아편수용체의 탈감작화(desensitization); 세포막의 아편 수용체 숫자의 감소(down-regulation), 아편 수용체의 내 재화(internalization), 아편 수용체와 G 단백의 짝풀림(uncoupling). 둘째, cAMP 회로의 상향조정(upregulation); adenylyl cyclase 의 초감작(supersensitization), 흥분성 G 단백질(Gs)과 아편 수용 체와의 짝지움(coupling). 셋째, 여러 가지 protein kinase들의 활 성 변화; cAMP-dependent protein kinase (PKA), protein kinase C (PKC), Ca/calmodulin-dependent kinases (CaMK), G proteincoupled receptor kinases (GPRK), mitogen-activated protein kinases (MAPK). 넷째, 아편 수용체와 다른 흥분성 수용체와의 상호 작용; NMDA receptor. 아편계 약물의 작용과 내성에 대한 분자생물학적인 기전 Regulator of G-protein Signaling (RGS) 단백질과 아편 신호 전달 연세대학교 의과대학 재활의학교실, Department of Anesthesia and Perioperative Care, University of California, 연세대학교 의과대학 마취통증의학교실 및 마취통증의학연구소" @default.
