- W2052189002 abstract "The aim of the study was to compare the diagnostic performance of risk of malignancy index RMI-I with the risk of malignancy algorithm and tumor markers CA125 and HE4 in the preoperative differentiation between malignant and non-malignant adnexal masses. Prospective study of 140 patients with adnexal masses admitted to Clinic for surgical operation. An ultrasound examination was done for each patients. Formula of Jacobs was used for calculation of risk of malignancy index RMI-I depending on the ultrasound score, menopause status and the serum concentration of CA125. The following ultrasound features were considered in the formula: the presence of multilocular cyst, presence of solid component, evidence of metastases in ultrasound scan, ascites and bilateral lesions. Serum concentration of CA125 was used directly in the formula. Menopause was defined as absence of menstruation for at least 12 months. Risk of malignancy algorithm ROMA was calculated depending on serum levels of CA125 and HE4. The definitive diagnosis of the adnexal pathology was confirmed by the pathological examination of the excised lesions. Malignant disease was confirmed in 25 cases (17,86%) and non-malignant disease in 115 cases (82,14%). 59 patients (24,14%) were after menopause. The specificity of RMI I, CA125, HE4 and ROMA was 97,39%, 73,04%, 93,04% and 88,70% respectively. The positive predictive value was 88,46%, 42,59%, 74,19% and 63,89% respectively. The negative predictive value was 98,25%, 97,67%, 98,17% and 98,08% respectively. The sensitivity of all the diagnostic tests was 92 %. RMI-I has higher specificity, positive and negative predictive values than CA125, HE4 and ROMA. There was no difference in sensitivity of these diagnostic methods." @default.
