- W2052521077 abstract "The Letter to the Editor from Price and Ayliffe is timely and raises some important issues about the use of control measures to contain or eliminate pathogenic micro-organisms in hospital settings. 1 Price E.H. Ayliffe G. Hot hospitals and what happened to wash, rinse and dry? Recent changes to cleaning, disinfection and environmental ventilation. J Hosp Infect. 2008; 69: 89-91 Abstract Full Text Full Text PDF PubMed Scopus (6) Google Scholar There are two main schools of thought on the value of using disinfectants on non-critical surfaces – one supported by Rutala and Weber, who are in favour of using disinfectants on non-critical surfaces, and another, mainly of European origin, that supports the use of cleaning rather than disinfection. 2 Rutala W.A. Weber D.J. The benefits of surface disinfection. Am J Infect Control. 2004; 32: 226-231 Abstract Full Text Full Text PDF PubMed Scopus (56) Google Scholar , 3 Dettenkofer M. Spencer R.C. Importance of environmental decontamination – a critical view. J Hosp Infect. 2007; 65: S55-S57 Abstract Full Text PDF PubMed Scopus (63) Google Scholar The overall question as to whether or not to use disinfectants is complex and both arguments have some merit. Surfaces, notably those proximal to patients, have been implicated in the spread of microbial pathogens, although it has been well-recognised that hand hygiene is the most effective measure for preventing hospital-acquired infections (HAIs). 4 Boyce J.M. Potter-Bynoe G. Chenevert C. King T. Environmental contamination due to methicillin-resistant Staphylococcus aureus: possible infection control implications. J Hosp Infect. 2001; 48: S40-S46 Abstract Full Text PDF PubMed Scopus (213) Google Scholar The now well-established use of alcoholic hand rubs (AHRs) has improved compliance and resulted in a decrease in HAIs where compliance is high. However, if compliance is high, the antimicrobial efficacy of AHRs might be limited, especially with short exposure time. We recently observed staff at Welsh intensive treatment units (ITUs) and the average contact time for using AHRs was between 10 and 15 s. It is worth noting that none of the products used in the Welsh ITUs recommended a contact time. With such a short exposure time, the efficacy of these antiseptic products is limited; we noted survival of MRSA isolates following exposure to several AHRs used in these hospitals (K.E. Cheeseman and J-Y. Maillard, unpublished observations). Thus the use of antiseptic products must be linked with compliance and appropriate training." @default.
