Surveillance has a variety of purposes including monitoring trends, detecting outbreaks and patterns of disease transmission. We do this to try to develop prevention measures. In order to judge how surveillance is doing, we need to know what we are measuring. So what is gastrointestinal (GI) infection? Four descriptions of GI infection tend to be used interchangeably: food poisoning, infectious intestinal disease (IID), gastrointestinal infection and gastroenteritis. They are not the same thing. Food poisoning includes non-infectious causes of enteric symptoms. IID is characterized by diarrhoea and vomiting. GI infections include those infections that may gain entry via the GI tract but do not necessarily cause GI symptoms, and gastroenteritis can include a variety of other non-infectious problems. Another thing to remember about the surveillance systems is that cases only get into the system if sufferers see their GP. How many of those in the audience have recently had a diarrhoeal illness? Most people would probably not admit it, but there ought to be at least 1/5 during the course of the last year according to the IID study. How many went to the GP? None I suspect. We should bear in mind that from the point of view of clinical investigations, they very often do not alter clinical management. It does not matter to the GP whether the patient has Salmonella or Campylobacter, unless the illness is long or protracted, because the clinical management is the same. We should also bear in mind that having a positive laboratory result could be incredibly helpful. It will tell us what the patient has got. If it is negative, it tells us" @default.
