Career burnout is a substantial problem in emergency medicine which is poorly addressed by both emergency medicine organizations as well as emergency medicine group practices, hospitals and academic institutions. The finding of roughly 30% of respondents reporting burnout being a significant problem should send a shiver down the spine of every emergency physician as well as the public—all is not well. Equally concerning is the recently released 2008 emergency physician salary survey conducted by Daniel and Stern Associates which reports 55% of 824 responders have considered leaving the practice of emergency medicine.2Daniel Stern and Associates2008 Emergency physicians Salary and Benefit Survey, Pittsburgh, PA.http://www.danielstern.comGoogle Scholar Burnout within emergency medicine is a problem in need of immediate attention and solutions other than providing temporary remedies such as wellness booths at American College of Emergency Physician Scientific Assembly. The culprit is the intensity of most emergency department (ED) work environments—characterized by more and sicker patients, inability to get admitted patients out of the ED, shift work, increased regulatory documentation burdens and declining resources. Most hospitals, groups, etc, maintain the mentality of a 40-hour work week as standard in emergency medicine. Long gone are the days of reasonable patient volumes or the ability to take a nap during an overnight shift yet we as a profession have made no workload adjustment. Emergency medicine societies should seriously consider establishing guidelines regarding acceptable clinical workloads which should be well below the 40-hour work week. Burnout will remain a significant problem until we adjust what constitutes reasonable clinical workloads in emergency medicine.
