The key step in making this intervention possible is the availability of rapid HIV testing. Despite the US Public Health Service recommendation to provide rapid testing on all labor and delivery units,1 however, many institutions have not yet implemented this valuable diagnostic tool. A number of obstacles exist, including deciding whether to perform the test in the laboratory or at the site of care; ensuring staff availability to conduct the test around the clock; developing counseling procedures and informed-consent materials; and creating protocols to follow in the event of a positive test. In addition, funding for rapid testing as a routine measure can be problematic.These obstacles have been largely overcome in the state of Illinois, which has one of the most effective rapid-testing programs in the country.2 Illinois requires labor and delivery units and nurseries to offer HIV testing and has a dedicated team of clinicians to provide on-site rapid testing training as well as a “24/7 Illinois Perinatal HIV Hotline.”Guidance for setting up rapid-testing programs can be obtained from the Centers for Disease Control and Prevention.3 In addition, the National Perinatal HIV Hotline (1-888-448-8765) can assist clinicians nationwide, 24 hours a day, with rapid-test interpretation and treatment recommendations.There are few medical interventions that have as dramatic an impact as antiretroviral prophylaxis to HIV-infected pregnant women and their newborns.
