• W2055636461 abstract "SUMMARY Clinical, neurophysiological and neuropathological studies on a 13-year-old boy with moderate manifestations of malignant hyperthermia and on his parents, a 20-year-old brother and a 15-year-old sister revealed die following features:— 1 Signs of affection of the central nervous system after general anaesdiesia and during a test with a small dose of suxamedionium, taking the form of hypertonicity, hyperactive deep reflexes, positive plantar reflexes and apnoea. 2 Occurrence of spontaneous activity in electromyography of the patient, his mother, brother and sister in die form of fibrillation potentials, spontaneous motor-unit potentials and large polyphasic potentials, evidencing muscular hyperexcitability. 3 Degeneration and regeneration of the intramuscular nerve fibres, increased occurrence of end-plates and pronounced sprouting in biopsy specimens. 4 Significant increases in the serum levels of creatine phosphokinase in the parents, die patient and his brother and sister, and in addition in diree of the mother's sisters. On the basis of these findings, both a central and a peripheral mechanism causing the manifestations of malignant hyperthermia are discussed. ZUSAMMENFASSUNG Die klinische, neurophysiologische und neuropathologische Untersuchung eines 13-jährigen Knaben mit dem mittelschweren Erscheinungsbild einer malignen Hyperthermic, sowie seiner Eltern, eines 21-jährigen Bruders und einer 14-jährigen Schwester, brachte nachstehende Tatsachen zutage: 1 Zeichen eines Befalls des ZNS nach Allgemeinnarkose und während des Testes mit einer kleinen Dosis von Suxamethonium, der sich in Hypertonizitat (der Muskulatur), hyperaktiven tiefen Reflexen, positiven Plantarreflexen und Apnoe manifestierte. 2 Auftreten einer Spontanaktivitat im Elektromyogramm des Patienten, seiner Mutter, seines Bruders und seiner Schwester in der Form von Fibrilla-tionspotentialen, spontanen »motor-unit«-Potentialen und breiter polyphasi-scher Potentiale, die auf eine Übererregbarkeit der Muskulatur hinwiesen. 3 Degeneration und Regeneration der intramuskulären Nervenfasern, vermehrtes Vorkommen von Endplatten und ausgesprochenes Sprossen in den bioptischen Proben. 4 Signifikante Anstiege des Serumspiegels der Kreatinin-Phosphokinase bei den Eltern, beim Patienten, seines Bruders und seiner Schwester, und zusätzlich auch noch bei drei Schwestern der Mutter. Auf der Grundlage dieser Befunde wird sowohl ein zentraler als auch ein peripherer Mechanismus als Ursache des Auftretens einer malignen Hyperthermic diskutiert." @default.
