- W2056019210 abstract "There has been increasing evidence that medical errors arise from poor communication between health care providers. The American Medical Association endorses the use of the Problem Oriented Medical Record (POMR) to facilitate communication. Yet, the therapeutic sections of this method are typically taught and practiced by haphazard means during clerkships. Currently it is unknown to what extent physicians and students associated with the Leiden University Medical Center structure their patient reporting based on the POMR. Thus we created a criteria form that rates different sections of the POMR and compared the results of pre-clerkship students after they had learned the principles of the POMR to those of local physicians. (See Figure) 147 students were assessed using a practice case in their last course prior to clerkships versus 55 of the planned 80 discharge letters sent from specialists to a local practitioner have been assessed to date. The interim results show that students are able to write a POMR note at a comparable (or better) level to a physician when finishing their didactic study. Both groups do well in diagnostic and poor in therapeutic sections. These results indicate that medical students should either learn & practice the therapeutic sections of the POMR more in their didactic training, or have a more structured clerkship training regarding this topic. However, the good scores on the diagnostic sections would indicate that the didactic training would be preferred.
