- W2056106425 abstract "The ability to communicate is cumulatively diminished in people suffering from Alzheimer's Disease (AD) and Vascular Dementia (VD). To date, a lack of appropriate assessment tools to measure dementia patients' ability to communicate has been experienced. Based on the current state of research in dementia care and social-communicative skills, KODEM, a standardized observational communication skills assessment tool has been developed and evaluated. The theoretical background of KODEM is based on the information-transfer-process model by Rüttinger and Sauer (2000) and the second basic axiom of Watzlawick, Beavin and Jackson (2007): “Every communication has a content and relationship aspect.” Initially, the interrater reliability was examined by videoratings (N = 10 dementia patients). Thereupon, six caregivers in six German nursing homes observed 69 residents suffering from dementia and rated their ability to communicate with KODEM. We used participant observation to provide an active observation of the subject in their natural environment. Furthermore, to verify the methodological quality, we complemented a standardized observation situation. Both observational situations span an interval of 10 minutes each. The subscale “Communication” of the Vienna List (Richter, 2003) was used to determine convergent validity of the ability to communicate. To establish discriminant validity, we assessed nursing care dependency by using the Barthel Index (Barthel, 1967). The interrater reliability of KODEM was excellent (mean kappa = .79; intraclass correlation = .91). Statistical analyses indicated that KODEM had excellent internal consistency (Cronbach's Alpha = .95). KODEM showed excellent convergent validity (r = .88) and discriminant validity (r = .63). Confirmatory factor analysis verified a two-factor solution: content aspect and relationship aspect. With regard of the aggravation of the disease, different trends were found concerning the content and relationship aspect of communication. While the ability to communicate verbally (content aspect) was continuously deteriorating, the ability to communicate using non-verbal methods (relationship aspect) persisted longer. KODEM is a reliable, valid and sensitive assessment tool to examine the ability to communicate in dementia patients. For practitioners it can be used to diagnose communication impairments and to improve communication and well-being in dementia. For researchers KODEM offers an in-depth examination of communication processes in dementia." @default.
