- W2056500996 abstract "In recent years, there have been impressive developments in lung transplantation. There are now successful and experienced programs throughout the world. In this issue of Seminars, a number of these recent developments are discussed. Strategies for lung preservation have improved and are, for the most part, standardized. This subject is discussed in detail by Drs. DePerrot and Keshavjee. In addition, the molecular events of reperfusion injury are better understood and, as a result, strategies have evolved to treat the patient with early graft dysfunction. The concept of non-heart-beating donors offers the prospect for increasing donor lung supply. Dr. Van Raemdonck and his colleagues have studied this subject extensively. Their discussion provides a detailed analysis of the practicality of using lungs from non-heart-beating donors. As application of lung transplantation has expanded, we are asked to evaluate and transplant patients with a variety of technical obstacles to transplantation. In addition, the donor population is not uniform. Therefore, a variety of technical modifications must be available to deal with the situation of the unconventional donor or recipient. Dr. Parekh and I outline our own strategies in the issue. Immunosuppressive strategies and agents have changed significantly in recent years. Dr. James Allen has a unique clinical and research perspective on the subject of immunosuppression relevant to lung transplantation. His contribution is a thoughtful review of this subject. The immunosuppressed patient is plagued by a risk of post transplant infection. The particular problems of post transplant viral infection are described in detail by Drs. Chakinala and Walter. Finally, the ultimate problem in lung transplantation, i.e., lung allograft dysfunction or bronchiolitis obliterans syndrome is thoroughly reviewed by Drs. Hachem and Trulock. Their program has an extensive experience with this problem. Long-term progress in lung transplantation will depend on a better understanding of the mechanisms of this devastating complication so as to evolve more effective treatment strategies." @default.
