- W2056544935 abstract "130 Laparoscopic donor nephrectomy (LDN) is associated with significantly decreased morbidity and inconvenience for the donor. Because of the technical demands of LDN and peritoneal insufflation, its application is thought to be limited to the non-obese donor. We hypothesized that obesity is not a technical contraindication to LDN. Beginning in 5-98, all living renal donation at Georgetown Transplant Institute has been performed using LDN. From 5-98 to 12-98, 30 patients underwent LDN. Evaluation included nephrology and psychiatric consultations. Anatomy was determined using MR angiography. LDN was performed with the transperitoneal technique and aggressive intraoperative volume loading. The kidney was extracted through a periumbilical or suprapubic incision. DVT prophylaxis consisted of venous compression stockings, low molecular weight heparin in obese patients and ambulation on the night of surgery. The following variables were assessed in a retrospective fashion: donor sex, age, weight, height, related vs. non-related donation, body mass index (BMI; wt/ht2), OR time, estimated blood loss (EBL), length of stay (LOS), time out of work (TOW), and complications. By definition, BMI>30 indicates morbid obesity, BMI>27 indicates >20% over ideal body weight, and normal BMI is 25. The patients were divided into non-obese (BMI≤30) and obese groups (BMI>30).
