- W2056565310 abstract "Prior studies have demonstrated the feasibility and safety of laparoendoscopic single-site surgery (LESS) in gynecology. Short term complications have also been previously reported. However, limited data exists regarding the umbilical morbidity with LESS. Our study objective was to determine the risk of umbilical hernia formation and other umbilical complications in a cohort of women undergoing LESS. Retrospective case review. Three academic hospitals. 174 women who underwent LESS for a variety of gynecologic procedures for benign and malignant gynecologic indications. Patients underwent a variety of LESS procedures. All patients returned for at least one visit 4-12 weeks postoperatively. Median follow up time was 12 months. Median age was 48; median BMI was 29. The majority of patients had at least one significant comorbidity. 2.4% of patients developed a postoperative umbilical hernia, all of which occurred in the “late” postoperative period, defined as greater than three months after surgery. Two patients had risk factors unrelated to LESS that may have been responsible for hernia formation. 7% of patients developed umbilical incisional cellulitis, and 0.6% of patients developed incisional hematoma at the umbilicus. No patients required readmission for complications. Obesity was found to be the only variable associated with umbilical complications on univariate analysis (p = .04); however, this was insignificant upon multivariate analysis. LESS does not appear to increase the risk of umbilical hernia formation, as umbilical hernia rates with conventional laparoscopy are comparable to the rates observed in this study. Surgeons should consider prolonged postoperative surveillance to identify latent umbilical morbidity, especially in obese women who may be at increased risk. Further prospective studies are needed to confirm the advantages and define the risks associated with LESS." @default.
