- W2056566606 abstract "Bariatric surgery has been proven to the most, if not the only, effective therapy for severe obesity (BMI >35kg/m2). Obesity is associated with endocrine alterations like subclinical hypothyreodism, male hypogonadism, and female hyperandrogenemia that improve after surgery-induced weight loss. While these endocrine improvements after bariatric surgery do not depend on the specific type of operation that is performed, changes in glucose metabolism and gastrointestinal hormone secretion critically depend on specific procedures. In general, bariatric procedures can be divided into purely restrictive procedures like gastric banding, purely malabsorptive procedures like biliopancreatic diversion, and combined procedures like gastric bypass. Malabsorptive and gastric bypass procedures in particular exert pronounced effects on the secretion of gastrointestinal hormones such as ghrelin, GLP-1, and PYY which are critically involved in the regulation of eating behavior. In line with these endocrine changes, patients frequently report reduced feelings of hunger after these bariatric operations. Furthermore, preliminary results from our laboratory suggest that gastric bypass but not gastric banding procedures distinctly reduce the hedonic value of eating that is commonly increased in severely obese subjects. These results strongly call for a further exploration of the influence of different bariatric procedures on the central nervous system. It can be expected that a large proportion of the beneficial effects are mediated by neuroendocrine mechanisms that have yet to be defined." @default.
