- W2056772384 abstract "You may wonder why we publish a clinical study of a new treatment without a control group; it is to illustrate the difficulties of carrying out a randomised trial of a new treatment of a chronic disorder of uncertain cause. The active ingredient of St John's Wort, Hypericum perforatum, is known to modify neurotransmitters in the brain and has been shown to be effective in treating mild depression. Depression may be a component of the premenstrual syndrome, which led Clare Stevinson and her colleagues (pages 870–876) to investigate the effects of Hypericum perforatum in this condition. Of the 96 women who underwent preliminary screening for the investigation 25 were enrolled, of whom 19 completed the study. Hypericum reduced the Daily Symptom Rating score by 49%, and there were significant improvements in the Hospital Anxiety and Depression Scale and the modified Social Adjustment Scale. These results seem impressive, so what are the problems? The premenstrual syndrome is a condition of fluctuating severity with a tendency to regression to the mean, where symptoms may resolve spontaneously; and in a clinical study of this nature there may be significant improvement due to a placebo effect. The authors suggest that the favourable results found in this investigation may have been more due to these factors than to a pharmacological benefit of Hypericum perforatum. This study reminds us of the intricacies of randomised trials in conditions of uncertain cause in gynaecological practice, such as chronic pelvic pain, chronic vulval pain and the premenstrual syndrome. However the results of this study are useful in designing a placebo-controlled trial of a new treatment of the premenstrual syndrome, and allow the investigators to make a reasonable estimation of the size of trial required." @default.
