- W2057254150 abstract "The concept of Vascular Cognitive Impairment (VCI) was introduced almost 20 years ago to encompass all the spectrum of cognitive sequels of cerebrovascular diseases, being dementia the last stage. Multiple etiologies may underlie cognitive impairment consequent to vascular diseases. Similarly to what is seen in Alzheimer's disease, predementia stages of vascular dementia exist. However, it is important to note that this may apply only to some subtypes with a progressive course. This is likely the case of small-vessel disease dementia where the increase in white matter changes and the accumulation of lacunar infarcts may lead to a progressive cognitive decline. Small vessel disease is considered the most frequent cause of VCI, homogeneous in clinical and neuroimaging terms and, therefore, suitable as a target for implementing studies and therapeutic trials. Apart from cognitive disorders, the clinical characteristics of VCI associated with small vessel disease are gait, mood, behavioural, and urinary disturbances. While in the final stage the patient fits the criteria for dementia, gait is very impaired, mood is altered with prominent depressive symptoms or apathy, and urinary incontinence is present, in the early phases, these disturbances can be mild and loosely associated. The LADIS Study has contributed with a great deal of information to the understanding of various aspects of white matter changes. Through this European multicenter collaboration, many clinical and functional outcomes in 639 elderly patients with subcortical vascular changes have been investigated. Subtle neurological signs have been documented to predict white matter changes severity. Moreover, the relationship between white matter changes and vascular risk factors has been explored. LADIS data have shown that the clinical and vascular risk factor profile may be suggestive of presence of cerebral small vessel disease. The concept of ‘brain-at-risk’ relates to the stage where patients with cerebrovascular diseases and vascular risk factors have no cognitive decline but are at increased risk thereof. Early recognition and detection is of crucial importance because the burden of cerebrovascular diseases on cognition can be effectively reduced by implementing preventive and therapeutic measures." @default.
