• W2057363086 abstract "Irinotecan (CPT-11), a camptothecin derivative, is the first of a new family of cytotoxic agents, the topoisomerase I inhibitors, to be introduced to the market. By inhibiting DNA topoisomerase I, CPT-11 interferes with DNA replication and provokes a single- followed by a double-strand break forcing the cell to enter into an apoptotic process. Although it displays potentially interesting activity in many tumours (e.g., lung cancers, cervical cancer), currently, its most consistent efficacy is in metastatic colorectal cancer where response rates compare favourably with those of 5-fluorouracil (5-FU), the only active drug for this indication over the past decades. The two principal adverse reactions of major concern are neutropaenia and a delayed and unpredictable diarrhoea. The mechanism underlying this latter reaction remains under active investigation and attempts are being made to improve the tolerability of CPT-11 treatment and to decrease the incidence of this side-effect. Nevertheless, CPT-11 can be administered on an out-patient basis, subject to careful monitoring for adverse events. Its unique mechanism of action makes CPT-11 a potentially interesting drug to test as one component of combination cytotoxic regimens.
