El tratamiento basado en quimioterapia, radioterapia o tratamiento neuroquirúrgico no se asocia con un aumento de la supervivencia pero sí con una mejoría de la sintomatología local y sistémica. Se describe el caso de un varón de 58 años con inicio de adenocarcinoma esofágico con panhipopituitarismo y diabetes insípida secundarios a metástasis hipofisaria. Se objetivó remisión posterior con respuesta al tratamiento poliquimoterápico. El tipo de tumor responsable así como la forma de inicio y la repuesta al tratamiento suponen un caso excepcional. Se describen las características epidemiológicas, clínicas, diagnósticas y terapéuticas de las metástasis hipofisarias. Metastases are an exceptional cause of pituitary mass and in some cases they can produce diabetes insipidus, or less frequently, panhypopituitarism. In 40-50% of cases, the pituitary metastases are the first evidence of an undiagnosed neoplasm. The most frequent sources of tumors causing metastases in the pituitary are the breast, lung and prostate. Treatment is based on chemotherapy, radiotherapy or neurosurgery but is not associated with increased survival, although local and systemic symptomatology is improved. We describe the case of a 58- years-old man with adenocarcinoma of the esophagus whose initial symptoms were panhypopituitarism and diabetes insipidus secondary to pituitary metastases with complete radiological (pituitary) and endocrinological remission after polychemotherapy treatment. The unusual primary tumor as well as the clinical manifestations and response to treatment make this case exceptional. The epidemiological, clinical, diagnostic, and therapeutic features of pituitary metastases are discussed." @default.
