- W2057560745 abstract "Urinary symptoms and sexual dysfunction are the two most common complaints following prostate radiation therapy. Factors contributing to if and how long a patient will experience urinary symptoms or sexual dysfunction has yet to be determined. We report our experience using CyberKnife stereotactic body radiation therapy (SBRT) in patients with organ confined prostate cancer and its effect on urinary symptoms and sexual health. This retrospective analysis includes one hundred two non-metastatic patients treated with CyberKnife SBRT at a single institution on an IRB approved protocol between May 2008 and August 2013. Thirty-seven were low-risk (T1b-T2A, Gleason <∖ = 6, and PSA < 10 ng/ml), fifty-six were intermediate-risk (T1c-T2b, and/or Gleason = 7, and/or PSA between 10.0 and 20.0), and eight were high-risk (T2c-T3, and/or Gleason 8-10, and/or PSA > 20). The prostate received 40 gy in 5 fractions of 8 gy. Intermediate and high-risk patients also had 36.25 Gy in 5 fractions delivered to the proximal seminal vesicles. Toxicities were recorded using the Common Terminology Criteria for Adverse Events. American Urological Association/International Prostate Symptom Score (AUA/IPSS) and Sexual Health Inventory for Men (SHIM) were recorded at baseline, 1, 3, 6, 9, 12, 18, 24, and 36 months after treatment. Median patient age was 71 years (range 48-93 years) and median prostate volume was 119 cc (range 64.7-239 cc). The effect of age and prostate volume on AUA and SHIM scores was recorded (Table 1). Acute toxicities were minimal with no grade 3 or higher toxicity reported. Prostate cancer patients treated with Cyberknife SBRT exhibited minimal acute toxicity. While an increase in AUA/IPSS score and a decrease in SHIM score initially occurred, patients resume normal activities immediately following treatment and the AUA/IPSS symptoms improve from baseline. Age did not affect urinary or sexual dysfunction following radiation therapy, while patients with a prostate volume < 110 cc had significantly better outcomes.
