- W2058528886 abstract "Radiology is only one method of adding to our knowledge of the mechanism at the cardia, but it is an important method, as it may reveal facts which cannot be discovered in any other way. For descriptive purposes it is convenient to start at the distal end of the gastro-oesophageal region and to work upwards into the oesophagus. A necessary preliminary, however, is a consideration of the correct positioning of the subject for examining this area. Positioning of the patient is of great importance, and is best understood if one imagines oneself looking down on the lower oesophagus as it moves forward from the spine (Fig. 1). It passes slightly to the left and traverses the hiatus. The part proximal to the hiatus is best shown in the right anterior oblique position. From the hiatus a segment of gullet passes to the left and this is well shown in the postero-anterior position. Creamer, Harrison and Pierce (1959) have called this portion of gullet, between the hiatus and the stomach, the “abdominal gullet”, which is a good name as there is some uncertainty as to how much of it is lined by gastric mucosa and how much by oesophageal mucosa. Finally, the notch between the abdominal gullet and the stomach, the “cardiac notch”, is slightly behind the cardia and, therefore, is only properly shown in the left anterior oblique position." @default.
