One of the most perplexing problems in medicine is to arrive at a diagnosis in a patient whose liver biopsy shows noncaseating granulomas in the absence of identifiable organisms. This is not an uncommon problem as reflected by a 3 to 10% prevalence of granulomas in liver biopsies performed at general hospitals (1-4).
