- W2058906863 abstract "The pulmonary endothelium, by virture of its immense surface area, is uniquely located to actively participate in the induction of inflammatory processes in the lung. A key component of the vessel wall, the vascular endothelium is a tissue that can be regulated; its function can be altered by a variety of diverse circulating vasoactive proteins, hormones, cytokines and inflammatory mediators. Attention initially was focused on cytotoxic effects of toxic agents such as oxygen radicals which were generated by leukocyte activation in proximity to the endothelium, produced by alterations in ambient oxygen concentrations or added directly to cultured endothelium. However, since extensive areas of denuded endothelium are never found in injured lungs even when superphysiologic concentrations of the injurious agents are utilized, the concept of endothelial cell activation and dysfunction emerged. Important functions of the normal endothelium include providing a nonthrombogenic surface and a selective barrier to circulating proteins and cellular components. Endothelial cell activation, however, promotes leukocyte adhesion and migration into the lung and profoundly disrupts pulmonary endothelial cell integrity producing pulmonary edema. The resultant barrier dysfunction has the net effect of producing organ dysfunction, tissue ischemia and dramatic changes in O2 transport. Attention now has focused on mechanisms of intracellular regulation, rather than injury, in the initiation and maintenance of pathogenic vascular syndromes in the lung, and endothelial cell activation-dysfunction is a central pathogenic event in a variety of lung disorders including ARDS, hypoxemic respiratory failure and pulmonary hypertension." @default.
