Essa ha permesso di ridurre le durate di ricovero e di migliorare il comfort postoperatorio del paziente, soprattutto se è associata a dei programmi di riabilitazione rapida, e, possibilmente, di ridurre l’incidenza delle complicanze parietali e delle aderenze addominali. Al di fuori dell’urgenza, il trattamento chirurgico di un cancro del colon sinistro è indicato al termine di un bilancio di estensione completo, che comporta una colonscopia diagnostica e una TC toraco-addomino-pelvica, preferibile all’ecografia epatica con radiografia del torace. La resezione oncologica tramite laparoscopia o laparotomia asporta un segmento colico passando minimo 5 cm a valle e a monte del tumore, dal momento che la totalità del mesocolon corrispondente con una legatura prossimale dei vasi permette una linfoadenectomia adeguata. È raccomandato l’esame di almeno 12 linfonodi regionali. Quando il tumore oltrepassa la sierosa colica (T4) e invade un organo adiacente, si raccomanda di eseguire un’exeresi in «monoblocco». L’approccio laparoscopico non è raccomandato in chirurgia d’urgenza per perforazione oppure occlusione su cancro colico, per dei tumori localmente avanzati (T4). La prescrizione di un’immunonutrizione preoperatoria di 5-7 giorni è raccomandata in tutti i pazienti in caso di chirurgia digestiva oncologica maggiore. Essa va proseguita nel periodo postoperatorio nei pazienti denutriti nel preoperatorio per 1 settimana o fino alla ripresa di un’alimentazione orale che assicuri almeno il 60% dei bisogni nutrizionali." @default.
