- W2059085458 abstract "Introdcution: Endoclips have been used primarily with hemostatic goals, with excellent results. Endoclips can be used additionally to secure various endoscopic implants, close post-endoscopic mucosal resection (EMR) or polypectomy sites, close visceral perforation, as markers, etc. Patients and Methods: A total of 74 patients were treated with endoclips (Olympus Corp). Indications included temporary use of feeding tubes, management of esophageal cancer, management of benign esophageal strictures, closure of perforation after endoscopic removal of submucosal lesions, closure after large polypectomy (colonic 6, gastric 2), closure after endoscopic mucosal resection (gastric 3, duodenal 3, colonic 5), and as a marker for external beam radiation. The uses included: 1) Securing feeding tubes in the jejunum in 23, 2) As reference sites for stent placement in 16, 3) To secure expandable esophageal stents (Polyflex, Boston Scientific) in 9, 4) To close post-polypectomy sites in 8, 5) To close post-EMR sites in 11, 6) To close perforations in 2, and 7) To mark for external radiation or surgery in 5. Results: Successful use of endoclips occurred in 22 of 23 patients for securing feeding tube in the jejunum (96%), in 16/16 to mark the sites for metallic stent placement (100%), in 9/9 patients to anchor Polyflex stents (100%), in 6/8 patients after polypectomy (75%), in 8/11 after EMR (72%), in 2/2 to close perforation (100%), and in 5/5 to mark a site for other treatments. The total number of clips placed was approximately 222 (average 3 per procedure, range 1-7). Complications included endoclip dislodgement from the delivery device in 13/222 (6%), incomplete endoclip opening in 10/222 (4%), dislodgement after placement in 7/222 (3%), and inability to grasp the tissue in 17/222 (7.6%). Conclusions: 1) Endoclips can successfully be used for non-hemostatic purposes, 2) Their use is safe and effective, 3) Improvement to the endoclip is needed to avoid device-related complication particularly in its ability to grasp tissue and in its opening diameter." @default.
