We hypothesized that patients treated aggressively would have a survival advantage over those managed conservatively. METHODS Review of our database identified 157 patients diagnosed with T1 between 1990 and 2007 who had residual T1 on re-TURBT, defined as a repeat TUR within 3 months of initial T1 diagnosis. IRC was defined as RC performed within 3 months of re-TURBT. Patients not treated with RC within 3 months were regarded as treated conservatively. Patients were followed every 3-6 months. Seven patients died within 3 months of re-TURBT, thus landmark survival analysis was used. Kaplan-Meier curves were drawn to determine the difference in cancer specific survival (CSS) between the two groups. Univariate Cox proportional hazards regression model was used to test the association between treatment approach and CSS. RESULTS No differences in baseline clinical characteristics were observed between the two groups other than BCG treatment. This was an expected difference as patients treated conservatively were likely treated with BCG. The median follow up in event free patients was 2.8 years. Cancer specific mortality occurred in 16 patients during the follow up. There was no statistically significant association between immediate cystectomy and CSS (figure; hazard ratio: 1.15, 95%CI: 0.43, 3.09, p=0.8). CONCLUSIONS The non-significant result is not surprising given the low event rate and small sample. The results are compatible with IRC reducing cancer-specific mortality by a little more than half.
