- W2059770366 abstract "Estimation of fetal weight (EFW) in twin pregnancies is an important determinant of timing and mode of delivery. The objective of our study was to assess accuracy of fetal weight estimation in twins. Retrospective analysis of ultrasound examinations performed by certified physicians. All twin pregnancies had at least one ultrasound examination performed < 15 days before delivery. Mean percentage error was defined as: (EFW-birth weight (BW))/BW × 100. Absolute error was defined as predicted weight-birth weight. Five formulae were used to estimate fetal weight: Hadlock1 (DBP, AC, FL), Hadlock2 (DBP, CC, AC, FL), Shepard, Ong and Warsof. Bland-Altman plot was performed to assess reliability. 268 twin pairs were included: Hadlock2 formula had the highest proportion of newborns with BW within 10% of EFW (63.4%, P < 0.001 vs the others; ns vs Hadlock1). We found mean errors within + /− 21% in 95% of the population using Hadlock formulae (33% for Shepard, 29% for Ong and 29% for Warsof; P < 0.001). When inter-twin discordance was > 25%, absolute error was higher (−46.2 ± 169.1 vs − 6.8 ± 212.9; P = 0.041). However the mean percentage error was not different (−1.7 ± 10.6 vs − 0.2 ± 10.8; P > 0.05). Estimation of fetal weight within 10%, 15% or 25% of birth weight was similar in both discordant and concordant fetuses. Ultrasound prediction of BW is less accurate in twins than in singletons. Although most Hadlock's formulae estimations were within 10% of real birth weight, random errors are more important in twins than in singletons and may limit the reliability of EFW." @default.
