- W2059792372 abstract "Summary Clinical experience suggests that adrenaline is beneficial during CPR, and experimental studies have demonstrated unequivocally that adrenaline increases coronary and cerebral perfusion pressures and flow during CPR through α-adrenergic mechanisms. The adrenaline dose most often used clinically does not have a strong rationale, and both experimental studies and preliminary clinical experience suggested that higher than usual doses might improve vital organ perfusion and increase the likelihood of successful resuscitation, particularly after a prolonged period of circulatory arrest. However, several major clinical trials failed to demonstrate significant improvement with high-dose therapy. The limited effectiveness of adrenaline in any dose may be in part the result of the adverse effects of β-adrenergic stimulation on myocardial oxygen requirements during ventricular fibrillation. However, there is so far no convincing evidence (or strong reason to believe) that alternative vasoconstricting agents with less prominent or absent β-adrenergic activity will be more efficacious. Endogenous β-adrenergic stimulation of the heart results in high myocardial oxygen requirements during ventricular fibrillation, even in the absence of pharmacological intervention, and available evidence suggests that significant improvement in the balance between myocardial oxygen supply and demand during resuscitation from ventricular fibrillation is unlikely unless myocardial oxygen requirements are reduced. There is reason to believe that non-selective β-adrenergic blockade might be effective in this regard, although the clinical role, if any, for interventions aimed at reducing myocardial oxygen requirements during CPR remains to be determined." @default.
