- W2059946700 abstract "Treatment results from radiation therapy remain poor for patients with inoperable or unresectable non–small cell lung cancer (NSCLC). The purpose of this study was to exam the factors associated with overall survival and determine the effect of treatment factors such as chemotherapy and radiation dose in patients with NSCLC. Patients with unresectable or inoperable Stage I to III NSCLC and enrolled in prospective clinical trials were eligible for this study. All patients received definitive radiation therapy (RT) with or without concurrent weekly carboplatin and paclitaxel. The primary endpoint of this study was 3-year overall survival. Survival outcomes were compared by Cox regression analysis between patients enrolled in image study and patients enrolled in a treatment protocol treated with radiation dose escalation. A total of 51 patients enrolled, 44 of whom with follow-up of > 6 months were included in this analysis. Median age was 68 (44–85) years. The majorities of patients were males with a KPS score of 80–90s and had less than 5% weight loss. There were 10 Stage I, 6 Stage II, and 28 Stage III diseases. The median total radiation dose was 65.4 Gy (45.0–81.4 Gy). The survival rates at 1/2/3 years were 100%/87.5%/52.5%, 53.3%/53.3%/53.3%, 81.2%/64%/42.6%, for Stage I, II and III diseases, respectively. Under univariate analysis, age (p = 0.103), gender (p = 0.301), Stage (p = 0.502), and chemotherapy (p = 0.664) were all not while radiation dose was significantly (p = 0.01) correlated with overall survival. There was no significant difference (p = 0.93) in overall survivals between patients enrolled in the image study (Stage I–III) and patients in the dose escalation study (all Stage III). Multivariate analysis confirmed radiation dose as a significant factor (p = 0.01). For patients with Stage III disease treated with concurrent chemoradiation, patients received ≥70 Gy had better overall survival than those with <70 Gy (p = 0.03).
