We sought to describe quality of life (QOL) in men of all ages and evaluate the effect that treatment for prostate cancer has on their quality of life by age. METHODS Data were extracted from the Cancer of the Prostate Strategic Urologic Research Endeavor (CaPSURE). 3,354 patients in the analysis had localized disease and completed pre- and post-treatment quality of life assessments by the UCLA prostate cancer index (PCI) and the SF-36 survey. A clinically meaningful decline in QOL was defined as a decrease of at least half a standard deviation from pre- to 2 year post-treatment values. P-values <0.01 were considered statistically significant. RESULTS The percentage of patients who had a decline in QOL was highest for sexual function (56%), sexual bother (51%), and urinary function (50%), while a minority of patients noted worsening in urinary bother (30%), bowel function (24%), and bowel bother (24%). Only 17% and 21% of patients had a decline in physical function and mental health scores, respectively. Age at diagnosis was significantly associated with several QOL domains in logistic regression adjusted for diagnostic year, ethnicity, CAPRA clinical risk, and number of comorbid conditions at diagnosis as shown in Figure 1. Increasing age was associated with decline in urinary function in brachytherapy patients (OR 1.04 (95%CI 1.01-1.07)). Older RP patients were more likely to have declines in physical function (OR 1.05 (1.03-1.08)) and less likely to have worse sexual bother (OR 0.98 (0.96-0.99)). Older primary ADT patients were less likely to have worsening of urinary bother (OR 0.92 (0.86-0.97)) and sexual bother (OR 0.91 (0.86-0.97)). CONCLUSIONS Our data show that changes in quality of life after treatment for prostate cancer vary by age and treatment type. Increasing age is related to QOL outcomes at 2 years after treatment for some QOL measures. However, older patients do not necessarily experience the largest declines in QOL in all areas.
