- W2060691237 abstract "Prevalence of hallucinations in Alzheimer's disease (AD) is 10 to 15%. Brain structures responsible of such behavioral modifications remain unknown in AD. In LBD, decrease perfusion in left posterior cingulate and precuneus correlates with increase hallucinations (O'Brien, 2005). Patients with LBD and Parkinson's disease have greater gray matter (GM) loss in the right inferior part of the frontal lobe (Sanchez-Castaneda et al., 2010). We propose to investigate In this work the phenomena of hallucinations during AD using voxel-based morphometry (VBM) on data (both MRI and FDG-PET) from the Alzheimer's Disease Neuroimaging Initiative (ADNI). MCI/AD patients with hallucinations, based on the NPI evaluation, were compared to MCI/AD patients without hallucination matched for age, sex, educational level, laterality and MMSE. Thus, for the MRI analysis, 43 patients with hallucinations (AD-hallu) were compared to 43 control patients (AD-c), and for the FDG-PET analysis 21 AD-hallu were compared to 21 AD-c. Brain MRI volume (GM and White matter, WM) and brain PET were used to compare AD-hallu from AD-c according to the method for VBM with SPM5 and Matlab 7.7 software. We also performed voxel-level correlations between GM/WM concentration, brain metabolism, and hallucinations intensity with SPM5. Comparison of AD-hallu to AD-c on brain MRI showed a diminished volume of the right lingual gyrus in the AD-hallu group. Comparison on PET showed an hypometabolism of the right frontal lobe -particularly in the orbito-frontal part-, and the right anterior cingulate gyrus. Moreover, hallucinations’ intensity were correlated to the diminished volume (MRI) of the vermis, the bilateral precentral gyri (Brodmann Area 6 = BA6) and the right superior parietal lobule and precuneus (BA7), and to hypometabolism of the right cingulate gyrus (anterior and posterior). Hallucinations in AD seem to be correlated to the following changes: first, an atrophy of regions known to be involved in visual treatment color/shape discrimination regions (lingual gyrus), in self consciousness (precuneus), and in reasoning -Kalbfleish et al., 2007- (right BA6 and BA7, cerebellum, lingual gyrus); second a diminished activity of regions of inhibition (right frontal lobe) and regions of limbic connexion (cingulate gyrus)." @default.
