- W2060838861 abstract "To the Editor.— The recent article on occult rupture of the spleen (236:2884, 1976) perpetuates the incorrect clinical lore that 15% of patients with splenic trauma have splenic rupture. The authors state that delayed splenic rupture occurs when bleeding recurs in the subcapsular hematoma, producing increased pressure and disrupting an already thinned, stretched capsule. The published reports of Olsen 1 and of Benjamin et al 2 have conclusively demonstrated that rupture of the spleen is an extremely rare clinical circumstance. In the vast majority of instances, splenic rupture is not delayed, but rather the recognition of splenic injury is delayed. The data of those authors 1,2 come from institutions in which diagnostic peritoneal lavage is routinely used in initial evaluation of suspected blunt abdominal trauma.
