- W2060899993 abstract "In the fetal vascular system, 3D/4D modalities such as high definition power Doppler, HD power flow Doppler, and B-flow, as well as advances in spatial and axial resolution of ultrasound machines, have made possible the study of smaller vessels not previously amenable to imaging. The fetal portal system is crucial to normal liver development. Until recently only larger vessels of this system were consistently correctly visualized, and have been studied. To determine which portal system blood vessels can be consistently visualized in addition to the major vessels, and to characterize their Doppler waveforms. Gravidae booked for fetal organ scans or routine evaluation (22–35 wks), with singleton fetuses without known anomalies or poly-or oligohydramnios were scanned for evaluation of portal system vessels. Vessels attempted: the main, left, right portal veins (MPV, LPV, RPV), splenic vein (SV), superior, inferior mesenteric veins (SMV, IMV), gastric vein (GV), and in addition, the celiac trunk (CTr), superior mesenteric artery (SMA), and splenic artery (SA). 311 gravidae were examined during the study period, from GA 22–35 (mean 28.6). The MPV, LPV, RPV, and SV were imaged in 97%, 100%, 100%, and 97% of fetuses, respectively, and their waveforms were continuous or monophasic. The SMV was imaged in only 8%, while the IMV and GV were not imaged satisfactorily; waveforms were not evaluated. The CTr, SMA, and SA were imaged in 100%, 100%, and 97% of fetuses, respectively. During the study 1 case of agenesis of PV was diagnosed, with no RPV and only proximal section of the LPV. The SV drained directly into one of the hepatic veins. Ductus venosus was intact. Part of the portal system is readily visualized, but only the SV, among those draining into the portal vein, is consistently available for Doppler flow evaluation, making it the preferable vessel for evaluation of portal system anatomic anomalies and flow dysfunction." @default.
